• You must be a Franklin County resident.

• Your child must be age 5 on or before August 1.

All students entering Kindergarten must present the following information to enroll in school:

1. Birth Certificate

2. Kentucky Dental Exam

3. Kentucky Physical Form dated within the past 12 months

4. Kentucky Immunization Certificate

5. Kentucky Eye Examination Form

6. Proof of Residence (any bill, lease agreement, etc.)

7. Proof of Guardianship (if child does not live with natural parents
